Congratulations on our pregnancy and thank you for your application.

We are honoured you have requested we be a part of your journey through pregnancy, labour, and post-partum.

Midwifery care spaces are based on your due date and the availability of a midwife for that date.  Only those applicants who we can accept into care will be contacted to book an initial appointment.  If you do not hear from us, please do not re-apply.  We will place your application on our waitlist in the event a space opens up in the future.  Please continue with prenatal care with your family care provider or walk in until you are accepted to care. 

If after you have submitted your application, you are accepted to another midwifery practice, would like to be removed from our waitlist, or your due date changes from your original application, we would love to know.  Please email admin@forestcitymidwiferycare.ca so we can update our waitlist to accommodate as many applicants as possible.

Please note that Womancare Midwives’ catchment area is the City of London, there may be another midwifery practice that services your area of residence if you live outside of London.  Please check www.aom.on.ca and click on find a midwife for information on practice locations and catchment areas.
